Home Modification Plan

There are several ways to modify your home care setting. You can do it yourself or hire a contractor. If you decide to use a contractor remember:

  • Know what you want done
  • Get recommendations
  • Check out the recommendations
  • Get three estimates and make comparisons
  • Put detailed contract in writing

In the previous section on home risks and hazards you have identified potentially dangerous situations and features in your home care setting. Now, prepare a plan for modifying your home care environment. Address each of the risks and hazards identified earlier. Write your suggestions in the template below. You can also print out a pdf version of this template and fill it as you tour the home.

After filling in the template, print it out. On the following pages you will have a chance to compare your suggestions with recommendations for home modification prepared by our Senior-Friendly Home Care team. We have identified home modification tips for people with visual, hearing, and mobility impairments.
